Good Faith Estimate
At Universus Physical Therapy, you have the right to receive a “Good Faith Estimate” that explains the cost of your medical care.
Under the law, healthcare providers are required to give patients who do not have insurance or are choosing not to use insurance an estimate of the total bill for medical services and items.
You are entitled to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes costs related to medical tests, prescription drugs, equipment, and hospital fees.
Your healthcare provider should give you a Good Faith Estimate in writing at least 1 business day before your scheduled service or item. You can also request a Good Faith Estimate from Universus Physical Therapy or any other provider before scheduling your service.
If you receive a bill that exceeds the Good Faith Estimate by $400 or more, you have the right to dispute it. Be sure to save a copy or take a picture of your Good Faith Estimate for reference.
